CHNA-CHIP

The Boston Community Health Collaborative brings together institutions and organizations contributing to the health and well-being of residents in Boston to build a shared understanding of the most important health needs, strengths, and priorities through a coordinated city-wide Community Health Needs Assessment and by developing and implementing a Community Health Improvement Plan.

Our Vision: A healthy Boston with strong communities, connected residents and organizations, coordinated initiatives, and where every individual has an equitable opportunity to live a healthy life.​

Our Mission: To achieve sustainable positive change in the health of Boston by collaborating with communities, sharing knowledge, aligning resources, and addressing root causes of health inequities.​

About Our Process

About the Community Health Needs Assessment (CHNA)

Every three years the Boston Community Health Collaborative brings together partners from across the local public health system, including community organizations, health systems and hospitals, community health centers, local government, social services, and others to review key health data trends and hear directly from residents and community leaders about the most important strengths, challenges, and priorities for improving the health of our city. The information gathered from this process informs our shared public health planning and guides the work toward making Boston a healthier city for all. Boston Public Health Commission is the central coordinating organization for the Boston Community Health Collaborative.  

 

About the Community Health Improvement Plan (CHIP)

The Community Health Improvement Plan (or CHIP) is a community-initiated action plan to address the key priority areas identified through our coordinated, city-wide Community Health Needs Assessment. The purpose of the CHIP is to set priorities, coordinate and target resources, and define actions that organizations and institutions can take within the local public health system to improve the health of our city. The 2022-25 CHIP recognizes the importance of  “upstream” social determinants of health in shaping people’s health, well-being, and quality of life.

Our Priority Areas

Based upon the findings of our coordinated Community Health Needs Assessment and Improvement Planning process, the 2022-25 Community Health Improvement Plan identifies four priority areas to focus our collective efforts toward a healthier, thriving city:  

  • Access to Services  

  • Economic Mobility & Inclusion  

  • Housing  

  • Mental and Behavioral Health

Community Engagement Efforts: ​Between May 2023 – December 2023, the Boston Community Health Collaborative partnered with the Boston Public Health Commission to co-host a series of community events to connect residents of Boston with health resources, share key findings and priorities from the Community Health Needs Assessment and Community Health Improvement Plan, and provide a forum to hear from residents about they would like to see their local public health system supporting in building a healthier, thriving city for all.  

BCHC staff additionally engaged with residents through community meetings and forums hosted by the Allston-Brighton Health Collaborative and the Mayor’s Office of Immigrant Advancement.

Thank you to all the partner organizations who supported this community outreach, including Codman Square Health Center, East Boston Neighborhood Health Center, Boston Centers for Youth & Families, Mattapan Community Health Center, Union Capital Boston, and Whittier Street Health Center.
