What to Know About the QUESTIONNAIRE

Organization Details: There is an optional field to include a link to your organization’s active programming or event calendar. Please use this field to direct users to your most up-to-date offerings, if applicable.

Organization Description: This section can be used to share anything not captured in earlier sections — such as a more personalized overview, additional guidance, key insights, or even an inspirational quote.

Organizational Logo: The last step is an option to upload a logo image file. If your organization has multiple versions, please choose the version with minimal text. If you encounter any upload issues, you may email the file directly to lindsey.swett@boston.gov.
