BPS Saturday Clinic for Youth Employment Health Certificate

Attend a BPS Saturday clinic if you are a BPS student who needs a health certificate but have not received a physical in the last 12 months and do not have access to a physical appointment.

If you are aged 14 or 15 when you start work this summer, you will need a Health Certificate to apply for your Work Permit, one of the required onboarding documents for any job. 

To help Boston Public Schools (BPS) students fulfill the Health Certificate requirement, BPS is offering Saturday clinics (6/8, 6/15, and 6/22) for its students who have not received a physical in the last 12 months, do not go to a school with a school-based health center*, and could not schedule a physical with a PCP before the summer.

How to attend the clinic and get your appointment time: Complete a sign-up form to request for an appointment. Please note that the time of the clinic is not actually the 9-10am indicated above. A BPS team member will review your form submission and confirm your attendance and your appointment time within 48 hours of receiving your form submission

* If you are a BPS student in need of a physical and you go to a school with a school-based health center, please reach out to the health center to schedule an appointment there. 

** If you are a BPS student and have received a physical in the last 12 months but have questions on how to fulfill the Health Certificate requirement, please reach out to your guidance counselor, family liaison, PIC career specialist, school nurse, or the City of Boston Office of Youth Employment & Opportunity.
